Vaginal dryness is a painful symptom that many people may experience at some point during their lives. This symptom can be caused by a decrease in hormone levels, breastfeeding or certain medications especially allopathic.
Vaginal dryness commonly linked to menopause (this common condition of menopause called vaginal atrophy). Vaginal dryness is a painful symptom, it can cause pain during sitting, exercising, urinating and sexual intercourse.
Normally, vaginal lining is lubricated with fluid that helps keep it thick and elastic. Vaginal dryness happens when the tissues in the vagina are dry, thin and not well-moisturized.
Vaginal dryness occurs at any age. It’s most common in women or people assigned female at birth (AFAB) during or after menopause when estrogen levels decline. The hormone estrogen helps keep the vaginal lining moisturized and healthy. Low levels of estrogen cause the vaginal walls to become thin and dry.
Around 17% of people assigned female at birth (AFAB) age 18 to 50 report problems with vaginal dryness during sex, even before menopause takes place. Over half experience vaginal dryness after menopause.
A dry vagina may also cause:
- Burning and itching,
- Bleeding after sex (due to vaginal wall tissues breaking open),
- Soreness in vulva,
- Recurrent urinary tract infections (UTIs) or yeast infections,
- Polyuria,
- Low libido.
Less moisture in the vagina leads to less moisture in the vulvar area (external genitals). That’s why a lady can feel dryness or irritation.
Possible Causes
In many cases, vaginal dryness happens when estrogen levels decrease. This occurs naturally as we age or during menopause.
Low estrogen levels decline, the skin and tissues of vulva and vagina become thinner and less elastic, and vagina can become dry.
Certain health conditions or treatments for health conditions and can also cause vaginal dryness. Vaginal dryness can result from:
- Breastfeeding.
- Birth control pills or any form of hormonal birth control.
- Cancer treatments including chemotherapy and hormone therapy.
- Diabetes.
- Allopathic medications, including anti-estrogen medications (treating uterine fibroids or endometriosis), certain antidepressants and antihistamines.
- Oophorectomy (removal of ovaries).
- Sjogren’s syndrome (an autoimmune disorder that can cause dryness throughout your body).
- Not being sexually aroused.
- Using scented or perfumed soaps, sprays and washes around or in the vagina.
Precautions
Vaginal dryness is usually most apparent during sexual penetration. Without enough vaginal lubrication, the friction or rubbing during sexual intercourse can cause pain and discomfort. Take time before sex to make sure you’re fully aroused. Engage in foreplay with your spouse and try to relax. Using natural lubricants for example olive oil, coconut oil, sunflower oil etc can also help.
Diagnosing Vaginal Dryness
Doctors diagnose vaginal dryness based on patient’s medical history and a physical exam. To find the cause, ask your patient about her symptoms and any medications especially allopathic she take; and perform the following tests:
- Pelvic exam to view the inside of patient’s vagina, which may be thin, dry and red.
- Blood test to determine if hormone levels or a health condition are causing vaginal dryness.
Take a sample of patient’s vaginal discharge to rule out other causes or for signs of infection.
Allopathic Treatment for vaginal dryness
There are many treatments available for vaginal dryness and painful intercourse (dyspareunia) associated with vaginal dryness.
Medications for vaginal dryness
Medications work (prescription only) by either replacing or acting like estrogen in patient’s body.
- Low-dose estrogen cream, ring or tablet: for example, estradiol – 0.05 mg, 0.1 mg, 2 mg vaginal ring; 4 mcg, 10 mcg vaginal insert; 0.01% vaginal cream. These medications work by replacing estrogen in patient’s body. Creams and tablets are applied directly into the vagina using an applicator. Most are prescribed for daily use until relief, then used weekly as needed. Estrogen-containing rings are placed into the vagina for up to three months, then replaced.
- Ospemifene: This medication is called a selective estrogen modulator (SERM) and is taken by mouth. It acts like estrogen in your body and helps treat painful sex associated with vaginal atrophy.
- Dehydroepiandrosterone (DHEA): This is another medication that acts like estrogen in your body. It’s a vaginal suppository that helps with painful sex in menopausal people.
Talk to your healthcare provider about the risks and benefits of medication containing estrogen or estrogen-like substances. Estrogen may not be safe for people who’ve had breast cancer or who are at high risk of breast cancer.
Side effects of vaginal estradiol for Vaginal Dryness
The most common side effects of vaginal estradiol are:
- Headache,
- Back Pain,
- Breast pain and tenderness,
- Abnormal vaginal bleeding or spotting,
- Itching, irritation, or burning sensation in the vagina,
- Stomach cramps, bloating, nausea, vomiting, or diarrhea,
- Hair loss,
- Edema,
- Vaginal yeast infection.
The most serious side effects of vaginal estradiol are:
Blood Clots
Vaginal estradiol may cause blood clots in arteries or veins and could cause a stroke, heart attack, or venous thromboembolism, including retinal vein occlusion (a blood clot in the back of the eye), pulmonary embolism), or deep venous thrombosis (DVT).
Endometrial Cancer
If a lady using estradiol may be at higher risk for endometrial cancer (cancer in the lining of uterus).
Breast Cancer
Patient may be at higher risk for breast cancer if she uses vaginal estradiol (new lump in the breast or armpit; changes in the size, shape, texture, or temperature of the breast; nipple changes).
Ovarian Cancer 
Patient may be at higher risk for ovarian cancer if she uses vaginal estradiol (unusual vaginal discharge; belly or back pain; polyuria.
Gallbladder Problems
Using vaginal estradiol may cause gallbladder problems, including cholecystitis and cholelithiasis (gallstones).
Patient should visit the doctor right away if she has/feels any of the following symptoms:
- If the skin or the whites of the eyes turning yellowish in color (jaundice),
- Fever,
- Pain in upper stomach area,
- Stool clay colored.
Memory Problems
Using vaginal estradiol may increase the risk for dementia.
Increased Blood Pressure (Hypertension)
Vaginal estradiol may increase patient’s blood pressure or worsen it if she has a history of high blood pressure. Patient has to contact the doctor if her blood pressure increases while using this medicine (180 mm Hg or higher or the diastolic is 120 mm Hg or higher.
Fluid Retention (Edema)
Vaginal estradiol can cause fluid retention, also called edema. Fluid retention can cause serious issues in people with certain conditions, such as heart failure or kidney disease (chest pain, irregular heartbeat, shortness of breath, stretched or shiny skin, swollen body parts, such as the belly or ankles etc).
Severe Allergic Reactions
Vaginal estradiol may cause allergic reactions, which can be serious. Patient should stop using vaginal estradiol and get help right away if she has any of the following symptoms of a serious allergic reaction:
- Breathing problems or wheezing,
- Racing heart,
- Fever or general ill feeling,
- Swollen lymph nodes,
- Swelling of the face, lips, mouth, tongue, or throat (angioedema),
- Trouble swallowing or throat tightness,
- Itching, skin rash, or pale red bumps on the skin called hives,
- Nausea or vomiting,
- Dizziness, feeling lightheaded, or fainting,
- Stomach cramps,
- Joint pain.
Toxic Shock Syndrome
Toxic shock syndrome may happen in people using an estradiol vaginal ring. Estradiol creates infections produces toxins that enters the blood. This triggers an immune response, which can be life-threatening. Remove your patient’s estradiol vaginal ring right away if patient have any of the following serious symptoms:
- Fever,
- Nausea, vomiting, or diarrhea,
- Dizziness or fainting,
- Muscle pain,
- A rash on the face and body that looks like a sunburn.
Vaginal Sores 
Sometimes using a vaginal estradiol ring can cause some of the lining of the vagina to get very thin, or for sores to develop. This can be very painful. Remove patient’s estradiol vaginal ring right away if she has any of the following symptoms of vaginal thinning or sores:
- Pain or soreness of the vagina,
- Vaginal bleeding,
- Swelling, redness, or scrapes in the vagina.
Blood Clots
Vaginal estradiol should not be used if patient have now or have a history of blood clots, such as a deep venous thrombosis (DVT), or your lungs, such as a pulmonary embolism (PE).
Breast Cancer
Vaginal estradiol should not be used if patient have or had previously had breast cancer.
Estrogen Dependent Cancer
Vaginal estradiol should not be used if patient have cancer that needs estrogen to grow for example ovarian cancer and endometrial or uterine cancer.
Stroke or Heart Attack
Vaginal estradiol should not be used if patient have had a stroke or heart attack.
Unusual Vaginal Bleeding
Vaginal estradiol should not be used if patient have abnormal vaginal bleeding.
Thrombophilia
Vaginal estradiol should not be used if patient have low blood levels of protein C, protein S, or antithrombin, or if patient have a condition that increases her risk of developing a blood clot.
Liver Problems
Vaginal estradiol should not be used if patient’s liver is not working as well as it should be. If there is a concern do tests to determine if it is working well enough to use this medicine.
Pregnancy
Do not prescribe vaginal estradiol to your pregnant patients.
Allergies to Ingredients
Patients who are allergic to any of the following should not use vaginal estradiol products:
- Estradiol,
- Estrace,
- Estring,
- Imvexxy,
- Vagifem,
- Femring,
- Yuvafem,
- Any of the ingredients in the specific product dispensed.
Side effects of ospemifene
Some side effects of ospemifene are:
- allergic reactions like skin rash, itching or hives; swelling of the face, lips, or tongue,
- breathing problems,
- breast tissue changes or discharge,
- signs and symptoms of a blood clot such as chest pain; shortness of breath; pain, swelling, or warmth in the leg,
- signs and symptoms of a stroke like changes in vision; confusion; trouble speaking or understanding; severe headaches; sudden numbness or weakness of the face, arm or leg; trouble walking; dizziness; loss of balance or coordination,
- vaginal bleeding,
- hot flushes or flashes,
- increased sweating,
- muscle cramps,
- vaginal discharge (white or clear) etc.
Side Effects of Dehydroepiandrosterone
When taken by mouth: DHEA has been used in doses of 50 mg daily.
Do not use DHEA in doses higher than 50-100 mg daily or for a long period of time. Using higher doses or using it long-term can increase the risk of serious side effects including cancer. DHEA is unsafe when taken during pregnancy or breastfeeding; it can cause higher than normal levels of androgen (male hormone), this is harmful to the baby. Do not advise dehydroepiandrosterone to patients of cancers (any type and any organ), diabetics, patients with hepatic diseases, depression and mood disorders, polycystic ovarian syndrome, patients with skin infections, in gastric diseases etc.
Natural lubricants
Natural oils such as grape seed oil, olive oil, sunflower oil, coconut oils may be a safe home remedy for vaginal dryness and should be used as an external lubricant before intercourse.

Argentum Nitricum
