Allopathic migraine and headache treatment is aimed at stopping symptoms and preventing future attacks.

Medications used to combat migraine and headache fall into two broad categories:

  • Pain-relieving medications. Also known as acute or abortive treatment, these types of drugs are taken during migraine attacks and are designed to stop symptoms.
  • Preventive medications. These types of drugs are taken regularly, often daily to reduce the severity or frequency of migraines.

Treatment choices depend on the frequency and severity of headaches, whether patient have nausea and vomiting with headaches, how disabling his/her headaches are, and other medical conditions.

Allopathic Medications for migraine and headache relief

Medications used to relieve migraine and headache work best when taken at the first sign of an oncoming migraine — as soon as signs and symptoms of a migraine begin.

  • Pain relievers. These over the counter or prescription pain relievers include aspirin or ibuprofen. When taken too long, these might cause medication-overuse headaches, and possibly ulcers and bleeding in the gastrointestinal tract, Hydrothorax, liver diseases etc. Migraine relief medications that combine caffeine, aspirin and acetaminophen may be helpful, but usually only against mild migraine pain.
  • Triptans. Prescription drugs such as sumatriptan and rizatriptan are used to treat migraine and headache because they block pain pathways in the brain. Taken as pills, shots or nasal sprays, they can relieve many symptoms of migraine. They might not be safe for those at risk of a stroke or heart attack, arteriosclerosis. pulmonary diseases (like pulmonary embolism, hydrothorax) etc.
  • Dihydroergotamine (D.H.E. 45, Migranal). Available as a nasal spray or injection, this drug is effective when taken shortly after the start of migraine and headache symptoms for migraine and headache that tend to last longer than 24 hours. Side effects can include worsening of migraine and headache-related vomiting and nausea, sneezing, arteriosclerosis, heart diseases, hypertension, kidney diseases including kidney failure, liver diseases and failure etc.
  • Lasmiditan (Reyvow). This tablet is approved for the treatment of migraine and headache with or without aura. In drug trials, Lasmiditan significantly improved headache pain. Lasmiditan can have a sedative effect and cause dizziness, so patients taking it are advised not to drive or operate machinery for at least eight hours.
  • Ubrogepant (Ubrelvy). This oral calcitonin gene-related peptide receptor antagonist is approved for the treatment of acute migraine and headache with or without aura in adults. It’s the first drug of this type approved for migraine and headache treatment. In drug trials, Ubrogepant was effective in relieving pain and other migraine symptoms such as nausea and sensitivity to light and sound two hours after taking it. Common side effects include dry mouth, nausea and excessive sleepiness, pulmonary, hepatic and renal diseases. Ubrogepant should not be taken with strong CYP3A4 inhibitor drugs.
  • CGRP antagonists. Ubrogepant and Rimegepant are oral CGRP antagonists approved for the treatment of acute migraine and headache with or without aura in adults. In drug trials, drugs from this class were effective in relieving pain and other migraine and headache symptoms such as nausea and sensitivity to light and sound two hours after taking it. Common side effects include dry mouth, nausea and excessive sleepiness. Patients with cardiac, renal, liver diseases should be careful with CGRP. Ubrogepant and Rimegepant should not be taken with strong CYP3A4 inhibitor drugs.
  • Opioid medications. For people who can’t take other migraine and headache medications, narcotic opioid medications might help. Because they can be highly addictive, these are usually used only if no other treatments are effective.
  • Anti-nausea drugs. These can help if patient’s migraine and headache with aura is accompanied by nausea and vomiting. Anti-nausea drugs include chlorpromazine, metoclopramide or prochlorperazine. These are usually taken with pain medications.

All of these allopathic medications are not safe to take during pregnancy, for renal, cardiac, hepatic and splenic patients. Don’t advise any of these medications as first choice.

Allopathic Preventive medications

Allopathically prevent frequent migraine and headache. The doctor might recommend preventive medications if patient have frequent, long-lasting or severe migraine and headache that don’t respond well to treatment.

All preventive medication is aimed at reducing how often a person get a migraine and headache, how severe the attacks are, and how long they last. Options include:

  • Blood pressure-lowering medications. These include beta blockers such as propranolol and metoprolol tartrate. Calcium channel blockers such as verapamil can be helpful in preventing migraine and headache with aura. (Patients with pulmonary diseases, asthma, liver and renal diseases should be careful with).
  • Antidepressants. A tricyclic antidepressant (amitriptyline) can prevent migraines. Because of the severe and deep acting side effects of amitriptyline, such as sleepiness, other antidepressants might be prescribed instead.
  • Anti-seizure drugs. Valproate and topiramate might help if patient have less frequent migraine and headache, but can cause side effects such as dizziness, liver function, renal function, weight changes, nausea and many more. These medications are contraindicated for pregnant women or women trying to get pregnant.
  • Botox injections. Injections of onabotulinumtoxin-A about every 12 weeks help prevent migraine and headache in some adults.
  • CGRP monoclonal antibodies. Erenumab-aooe, fremanezumab-vfrm, galcanezumab-gnlm, and eptinezumab-jjmr are newer drugs to treat migraine and headache. They’re given monthly or quarterly by injection. Common side effects include dry mouth, nausea and excessive sleepiness. Patients with cardiac, renal, liver diseases should be careful with CGRP. All side effect is still unknown as this drug is newer and is still under study.

Here are few medicines for migraine and headache treatment:

 Belladonna

Belladonna can be very helpful for migraine and headache with intense throbbing, pulsating headaches, an extreme fullness in the head, photophobia…Light worsen the migraine and headache, noise is unbearable.

In some cases, exposure to cold air can trigger a migraine episode. In others, a head bath with cold water can bring on migraine headaches.

Gelsemium works well when dim sight occurs with a migraine and headache. In some cases, temporary blindness precedes onset of headache. Spigelia

Fasting (going without meals) often triggers a migraine, headache starts in the morning, increases during the day and lasts until sunset, migraine during menopause.

Sepia

Sepia is the most suitable medicine for migraine and headache attacks in women around menopause. In such cases, terrible headaches accompany menopause. The headache gets worse indoors, open air brings relief. Nausea and vomiting may attend migraine headaches. Lying on the painful side of the head affords relief. The top of the head may feel cool in such cases.

In general, hot flushes, sometimes along with sweat, are well marked.

Cyclamen

Cyclamen is majorly indicated for migraine and headache attacks that begin with flickering, fiery sparks before the eyes or dim vision. These visual symptoms that precede the migraine headache are referred to as aura. Migraine and headache start on rising in the morning. Shooting pain is experienced in the temporal region. Vertigo, especially when standing, may accompany the headache.

Pulsatilla

In such cases pain is mostly throbbing at the top of head. They may also have irregularity of menstrual periods.

Ipecacuanha

Ipecac is a very helpful medicine for cases of migraine and headache with nausea and vomiting. Throbbing and heated sensation is felt in the head. In most cases, the pain can be present in the back of the head or top of the head.

Ruta Graveolens

An important medicine for migraine and headache caused by eye strain. Stitching pain is felt in the head from reading mostly where it is required.

Kali Bichrome

Managing migraine and headache preceded by blurred vision. The headache occurs in small spots and is marked over the eyebrows. Nausea and vomiting may accompany other symptoms.

Natrum Carbonicum

Other than Glonoinum, another medicine that shows quick and effective results in migraine and headache from sun exposure is Natrum Carbonicum.

Vertigo appears along with migraine and headache from sun exposure in such cases. The pressive migraine and headache is accompanied by cloudiness of eyes. Heat in the head and congestion may be felt. Shooting pain in the forehead also suggests use of Natrum Carbonicum.

Magnesia Phosphorica

Migraine and headache: pains shooting, darting, stabbing, shifting; intermittent and paroxysmal. During the night, severe throbbing pressure on vertex, l. side, deep in brain. Dull headache, as if brain too heavy (after protracted mental effort). Headache greater towards evening, but changes to a pressure above eyebrows, especially right.

Headache beginning in, or worst in occiput, and constant whilst attending school. Severe headache: face flushed, red; pain began in occiput, extended over whole head; sick at stomach; aches all over; better towards 9 or 10 a.m. to 4 or 8 p.m.

Pressive pain in head down through middle of brain. Pain through temples, top and back of head, with sensation of fullness, better lying down.
