Scleroderma is an autoimmune condition, which means our immune system attacks our bod’s own cells instead of protecting it. Experts don’t know the cause of immune system disorders. It’s like it can no longer tell the difference between what’s healthy and what’s not — between what’s our own and what’s an invader like bacteria or a virus.
Scleroderma is a chronic condition, which means with conventional/allopathic medicine patient will need to manage his symptoms for a long time (maybe the rest of his life). It can also cause life-threatening complications if it affects tissue in organs.
Types of scleroderma
There are two main types of scleroderma:
Localized scleroderma: Localized means concentrated in one area. Localized scleroderma only affects one part of the body (usually the skin). It causes thick patches or streaks on the skin that feel waxy. Localized scleroderma can get better (resolve) on its own. It usually doesn’t spread to other parts of the body.
Systemic sclerosis: Systemic sclerosis can affect other organs, in addition to the skin. It can affect parts of respiratory system and digestive system. Scleroderma is more likely to cause serious complications if it affects the ability to breathe or process nutrition. It can be fatal.
Systemic sclerosis has three subtypes — diffuse, limited and sine sclerosis.
1- Diffuse sclerosis
Diffuse means spread out widely. Diffuse sclerosis causes thickened skin over larger areas at once for example:
- Chest.
- Abdomen.
- Thighs.
- Arms.
- Legs.
- Face.
It can also affect multiple organs at once for example:
2- Limited sclerosis (CREST syndrome)
Limited scleroderma is the acronym CREST syndrome. Each letter in CREST stands for a symptom it causes:
- Calcinosis (extra calcium deposits in the skin).
- Raynaud’s syndrome (color changes and numbness in the fingertips and toes).
- Esophageal dysfunction (difficulty swallowing and acid reflux).
- Sclerodactyly (tight skin on the fingers).
- Telangiectasias (red or discolored spots on the skin).
3- Sine sclerosis
Sine sclerosis causes limited sclerosis symptoms but doesn’t affect the skin. Patient may have any CREST syndrome symptoms but not experience any thickened skin.
Symptoms and Causes
Some people with early scleroderma don’t have any symptoms. The most common scleroderma symptom is having patches or streaks of thickened, waxy skin. Symptoms are depending on which type of scleroderma patient have.
Other common symptoms include:
- Joint pain.
- Stiffness (especially when waking up in the morning).
- Fatigue.
- Unexplained weight loss.
Localized scleroderma symptoms
Patients with localized scleroderma usually only experience skin thickening. The thickened skin can be isolated to one specific area or appear in patches. It’s rare for localized scleroderma to affect your internal organs.
It can affect skin on:
- Chest.
- Abdomen (the area around your stomach).
- Arms and legs (your limbs).
- Hands and fingers.
- Feet and toes.
Systemic sclerosis symptoms
Systemic scleroderma can cause lots of symptoms. It causes thickened skin, usually in larger areas and patches, including on the face and hands. Thick skin usually appears on fingers or toes and then spreads toward the center of the body. If patient have Raynaud’s syndrome, the skin on the affected fingers and toes may change color when they’re exposed to cold (usually white, reddish or purple).
Systemic sclerosis can also cause symptoms in other organs and tissue for example:
- Muscles: Numbness and swelling, especially in hands and feet (extremities).
- Joints: Stiffness, swelling and loss of mobility.
- Lungs: Coughing and shortness of breath (dyspnea).
- Digestive tract: Difficulty swallowing. (dysphagia), heartburn, bloating, constipation and diarrhea.
- Heart: Abnormal heartbeats (arrhythmia), fluid buildup around your heart (pericardial effusion) and cardiac fibrosis.
- Kidneys: Kidney failure (which can be life-threatening).
- Genitals: Erectile dysfunction (ED) and vaginal dryness.
Scleroderma risk factors
Anyone can develop scleroderma, but some groups of people have a higher risk:
- People assigned female at birth (AFAB) are four times more likely than people assigned male at birth (AMAB) to develop scleroderma.
- People 30 – 50 years old — it’s rare for people younger than 30 to have scleroderma.
- Black people are more likely to have scleroderma, usually develop it earlier and are more likely to experience symptoms that affect their respiratory system. They also usually have more severe skin symptoms.
Complications
People with scleroderma are much more likely to have two other conditions: Raynaud’s syndrome and Sjögren’s syndrome.
Raynaud’s syndrome affects small blood vessels in the fingers and toes (digits). Patients with it have episodes/attacks of symptoms. It makes blood vessels in the digits suddenly tighten (contract) more than they should. This can make the skin in affected digits turn pale or lighter than natural skin tone. They might also look bluish.
Sjögren’s syndrome makes your body produce less moisture in certain glands — usually the salivary glands and the tear glands (that produce tears and keep eyes wet). Some people with Sjögren’s syndrome experience muscle and joint pain, too.
Some types of sclerodermas can cause severe complications for example:
- Kidney failure.
- Pulmonary hypertension (high blood pressure in the arteries that carry blood from your heart to your lungs).
- Pulmonary fibrosis (scarring in lung tissue).
- Cardiovascular disease.
- Congestive heart failure.
- A weakened immune system (immunodeficiency).
- Gastrointestinal diseases.
- Cancer.
Diagnosis and Tests
Start diagnosing scleroderma with a physical exam, advise some tests, refer to a rheumatologist, who specializes in treating autoimmune disorders. Ask your patient which symptoms he/she is experiencing, when he/she first noticed them and if anything seems to make them worse.
Tests required
Diagnosing scleroderma is usually part of a differential diagnosis. This means you will probably use a few tests to determine what’s causing the symptoms before ruling out other conditions and diagnosing with scleroderma.
Some tests you might need are:
- Antinuclear antibody (ANA) testing is the most important blood test to screen for scleroderma and other connective tissue diseases.
- Nailfold capillaroscopy can show if patient’s nails’ capillaries are larger than normal or bleeding slightly, all signs of vascular conditions caused by scleroderma (Stereo microscope, Videocapillaroscope, Dermatoscope).
- Skin Tests: Modified Rodnan Skin Score (Skin thickening, especially on the skin over the large knuckles).
- Blood tests (to see how well your immune system is working).
- Pulmonary (Lung) Function tests – to show if the lungs or respiratory system are affected.
- Pulmonary Arterial Hypertension (PAH) tests.
- Biopsy to remove a sample of affected skin or other tissue for lab.
- Endoscopy (looking inside the throat or stomach) if you’re experiencing gastrointestinal (GI) symptoms.
Imaging tests to take pictures of the inside of the body:
- Electrocardiogram (ECG).
- Echocardiogram (Echo).
- Right heart catheterization.
- A chest X-ray.
- A computed tomography (CT) scan.
Conventional/allopathic Treatment
In conventional/allopathic medicine, there’s no cure for scleroderma, but a doctor can help the patient find a combination of treatments that manages his/her symptoms and minimizes how much they impact patient’s daily routine.
Which treatments you’ll need depends on where patient experiencing symptoms and how severe they are. Some common scleroderma treatments include:
- Skin treatments: Creams and moisturizers to prevent the skin from drying out.
- Immunosuppressants: Immunosuppressants stop patient’s immune system from damaging healthy cells and tissues (be careful of severe side effects).
- Medicines to manage specific symptoms: For example, medication to manage the blood pressure, improve patient’s breathing, manage kidney failure or relieve gastrointestinal symptoms.
- Physical therapy: A physical therapist will help improve how patient’s body physically moves.
- Light therapy/phototherapy: Light therapy uses bright, focused UV light to treat skin conditions. It can help treat thickened skin.
- Stem cell transplants: Some patients with severe symptoms might need a stem cell transplant. A stem cell transplant helps body replace damaged blood cells with healthy donor cells.
