What are pediatric musculoskeletal disorders?

Pediatric musculoskeletal disorders are those that can impact one or more of the following tissues:

  • bones,
  • joints,
  • muscles,
  • connective tissue (ligaments, tendons, and cartilage).

All of these tissues help our body move smoothly. Because of this, musculoskeletal conditions often lead to reduced range of motion and sometimes pain.

Most common types of pediatric musculoskeletal disorders

There are many different types of musculoskeletal conditions. That means that there are also a variety of causes of pediatric musculoskeletal conditions. For example, some of these conditions may be related to structural issues that are present from birth or develop over time. Meanwhile, others may happen due to increases in inflammation in the body. I have divided up pediatric musculoskeletal conditions into a few broad categories. These are:

  • structural,
  • genetic,
  • inflammatory,
  • neoplastic.

 Structural disorders

Structural pediatric musculoskeletal disorders impact the structure of bones and joints. They may be congenital or develop as a child ages.

Here are some of the common structural pediatric musculoskeletal disorders:

Typically, our spine appears straight when viewed from behind. In scoliosis, the spine has a C-shaped or S-shaped curve when viewed from behind. Some signs of scoliosis include:

  • uneven shoulders or waistline,
  • one shoulder blade that sticks out more than the other,
  • one side of the ribcage that’s higher than the other side etc.

Scoliosis typically becomes apparent during puberty when children are undergoing growth spurts. It’s estimated to affect about 3% of children and adolescents.

Severe scoliosis curves can potentially lead to pain and difficulty breathing.

Clubfoot

Clubfoot is when the foot is turned toward the body, meaning that the bottom of the foot faces sideways or upward. If left untreated, it can lead to problems walking, painful calluses, and difficulty in everyday activities.

Developmental dysplasia of the hip

Developmental dysplasia of the hip (DDH) is another common pediatric musculoskeletal condition. Like clubfoot, it occurs in about one in 1,000 births.

DDH happens when the hip joint’s socket is shallower than normal. This means that the ball of the femur cannot fit into it, making the hip joint less stable.

Signs of DDH can include:

  • reduced mobility,
  • changes in gait, such as limping or toe-walking,
  • legs that are different lengths.

Slipped capital femoral epiphysis

Slipped capital femoral epiphysis (SCFE) is a hip condition that can occur during early adolescence. It’s estimated to affect 0.5 out of 1,000 children.

SCFE happens when the ball of the femur slips downward in relation to the rest of the bone. This can lead to symptoms like:

  • pain in the area of the hip, groin, thigh, or knee,
  • limping,
  • a foot or feet that are turned outward.

Some common genetic pediatric musculoskeletal disorders

Sometimes genetic changes can lead to pediatric musculoskeletal disorders. Muscular dystrophy is one of the common genetic causes of pediatric musculoskeletal problems.

Compared to the structural conditions that we just discussed, muscular dystrophies are rarer. They’re estimated to affect one in every 5,000 to 10,000 people worldwide and are often diagnosed in children.

Muscular dystrophies lead to progressive damage to muscles. This can cause:

  • muscle weakness that gets worse over time,
  • frequent falls,
  • changes in posture and gait,
  • increasing trouble with movement,

While there are many types of muscular dystrophy, Duchenne muscular dystrophy (DMD) is the most common childhood form. Because it follows an X-linked inheritance pattern, male children are most likely to be affected by it.

A couple of examples of other rarer genetic causes of musculoskeletal disorders in children include achondroplasia and osteogenesis imperfecta.

Conditions that lead to inflammation in the body can also cause musculoskeletal disorders in children. These often have infectious or autoimmune causes, for example:

Juvenile idiopathic arthritis

Juvenile idiopathic arthritis (JIA) is a type of arthritis caused by autoimmune disease. In JIA, the immune system mistakenly attacks joint tissue, leading to inflammation and damage. The symptoms of JIA can include:

  • joints that are:
    • swollen,
    • tender or painful,
    • warm to the touch,
    • stiff, especially if they’re in one position for a long time,
  • reduced activity and range of motion,
  • fatigue,
  • fever.

JIA can also affect other parts of the body, such as the eyes, skin, and lungs.

Osteomyelitis

Osteomyelitis is a bone infection. It often occurs due to trauma that impacts a bone and introduces microbes like bacteria. This can happen through fracture, injury, surgery, or previous infection. Osteomyelitis is estimated to affect 13 in 100,000 children.

If left untreated, the infection can spread or lead to lasting bone damage. The symptoms of osteomyelitis are:

  • severe pain,
  • swelling, redness, discoloration, and warmth in the affected area,
  • difficulty using the affected area, such as walking with a limp if a leg bone is affected,

Septic arthritis

Septic arthritis happens when an infection in another part of the body spreads into a joint, often the hips, knee, or ankle. It’s most commonly caused by bacteria. Septic arthritis is less common in children than the other inflammatory musculoskeletal disorders discussed above. It’s estimated to impact up to 4 in 100,000 children.

The symptoms of septic arthritis typically appear quickly and may include:

  • severe pain and swelling in the affected joint,
  • reduced range of motion,
  • fever,
  • chills.

What are common pediatric neoplastic musculoskeletal disorders?

Pediatric musculoskeletal disorders can also have neoplastic causes. The term “neoplastic” basically refers to an abnormal growth of tissue encompassing both benign (noncancerous) and malignant (cancerous) tumors.

The majority of pediatric tumors affecting the musculoskeletal system are benign. However, musculoskeletal tumors also represent some of the most common malignancies in children, including:

  • bone cancers like osteosarcoma and Ewing sarcoma,
  • rhabdomyosarcoma (a muscle cancer).

Cancers can also affect other types of musculoskeletal tissue, but they are less common, for example cancers that develop in tendons, cartilage, and joint tissue.

Symptoms of musculoskeletal tumors can vary by type but may include a lump that patient can feel and pain in the affected area as the tumor presses on surrounding tissues.

How are pediatric musculoskeletal disorders diagnosed?

In order to diagnose pediatric musculoskeletal disorders, first get a child’s medical history and do a physical examination. During the physical examination:

  • observe the appearance of a bone, joint, or surrounding skin,
  • feel for pain or tenderness,
  • assess the range of motion of a joint,
  • check reflexes,
  • evaluate muscle strength,
  • observe how a child walks.

Also do additional tests to help rule out other conditions and make a diagnosis. These tests includes:

  • blood and urine tests,
  • imaging tests, such as X-rays, CT scans, or MRI scans,
  • analysis of a biopsy sample,

Cocculus Indicus

Within the sphere of action of Cocculus are many spasmodic and paretic affections, notably those affecting one-half of the body. Affects the cerebrum, will not cure convulsive seizures proceeding from the spinal cord. Painful contracture of limbs and trunk; tetanus. Kyphoscoliosis and scoliosis.

Cracking of vertebrae. Paralytic pain spine. Pain in shoulder and arms. Pressure in scapula and nape. Stiffness on moving shoulders, spine. Lameness. Trembling and pain in limbs. Arms go to sleep. One-sided paralysis. Hands are alternately hot and cold and numbness. Lower limbs very weak. Inflammatory swelling of spine, knee. Intensely painful, paralytic drawing. Limbs straightened out, painful when flexed.

Capricious. Heavy and stupid. Inclination to sing irresistible. Slow of comprehension. Mind benumbed. Profound sadness. Cannot bear contradiction. Speaks hastily. Very anxious about the health of others.

Conium Maculatum

Conium is an excellent medicine for kyphoscoliosis and scoliosis, difficult gait, trembling, sudden loss of strength while walking, painful stiffness of joints of legs. debility, hypochondriasis, urinary troubles, weakened memory, sexual debility. Dorsal pain between shoulders. Ill effects of bruises and shocks to spine. Coccydynia. Dull aching in lumbar and sacral region.

Causticum

Manifests its action mainly in chronic rheumatic, arthritic and paralytic affections, indicated by the tearing, drawing pains in the muscular and fibrous tissues, with deformities about the joints, progressive loss of muscular strength, tendinous contractures. Broken down senile.

Nux Vomica

Backache in lumbar region. Burning in spine. Cervical-brachial neuralgia. Bruised pain below scapulae. Sitting and standing is painful. Kyphoscoliosis and scoliosis. Arms and hands go to sleep. Paresis of arms, with shocks. Legs numb; feel paralyzed; cramps in calves and soles. Partial paralysis, from overexertion. Cracking in all joints during motion. Drags his feet when walking. Strychninum Purum

Its primary function is to stimulate the motor centers and the reflex action of the spinal cord. Spasms of muscles, cramps from an undue reflex excitability of the cord, spasms of the bladder, etc. Kyphoscoliosis and scoliosis.

Stimulates the central nervous system, mental activities, special senses rendered more acute. Respiration increased. All reflexes are made more active. Stiffness in muscles, spine, face and neck. Opisthotonos – tetanic convulsions. Explosive nervousness. The pains and sensations come suddenly and return at intervals.

Spasm of muscles about larynx. Excessive dyspnea. Sharp, contractive pains in muscles of chest. Rigidity of cervical muscles. Sharp pain in nape and down spine. Oxalicum Acidum

Staggering gait. Loss of co-ordination. Patellar tendon reflex lost. Pains come and go quickly, but muscles remain sore and stiff.

Secale Cornutum

Produces contraction of the unstriped muscular fibers. Anemic condition, coldness, numbness, petechiae, mortification, gangrene. Debility, anxiety, emaciation, though appetite and thirst may be excessive. Facial and abdominal muscles twitch. Passive, congestive pain (rises from back of head), with pale face. Head drawn back. Falling of hair; dry and gray. Nosebleed, dark, oozing.

Deformed spinal cord. Spinal irritation, kyphoscoliosis and scoliosis, tingling of lower extremities; can bear only slightest covering. Locomotor ataxia. Formication and numbness. Myelitis. Veratrum Viride

Tiegel’s contracture, Thompson’s Disease, athetosis and pseudo-hypertrophic muscular paralysis present a symptomatology. Kyphoscoliosis and scoliosis. Difficult breathing. Cimicifuja Racemosa

Has a wide action upon the cerebrospinal and muscular system, as well as upon the uterus and ovaries. Especially useful in rheumatic, nervous subjects with ovarian irritation, uterine cramps and heavy limbs. Its muscular and crampy pains, primarily of neurotic origin, occurring in nearly every part of the body, are characteristic. Agitation and pain indicate it. Pains like electric shocks here and there.
