Hemolytic uremic syndrome (hee-mah-lit-ick yer-ee-mik sin-drohm) is a medical condition that blocks the small blood vessels in our kidneys. The blockage destroys your red blood cells (hemolytic anemia) and reduces the number of platelets (thrombocytopenia), which are clotting cells. Hemolytic uremic syndrome (HUS) most commonly affects your kidneys. E. coli is the most common cause of Hemolytic uremic syndrome (HUS).
Atypical hemolytic uremic syndrome
Atypical hemolytic uremic syndrome (aHUS) is a rare type of Hemolytic uremic syndrome. Its features include low levels of red blood cells, a low platelet count and acute kidney failure.
Atypical hemolytic uremic syndrome is typically a genetic condition. Mutations in some of our genes that encode proteins cause aHUS. Another illness, an infection or an environmental factor triggers the mutation to activate.
Who does hemolytic uremic syndrome affect?
Anyone can develop hemolytic uremic syndrome. However, a person is more likely to develop it if he/she has:
- Are under five years old.
- Have a weakened immune system.
- Have a family history of HUS.
- Get Escherichia coli (E coli) bacteria, usually from eating undercooked meat or milk of diseased cattle.
- Have direct contact with someone who has diarrhea due to E coli.
Symptoms and causes
Hemolytic uremic syndrome symptoms include:
- Diarrhea, usually bloody.
- Fever.
- Abdominal pain.
- Vomiting.
- Fatigue.
- Easy bruising.
- Pale skin or the linings of nose and mouth (mucous membranes) are pale.
- Confusion.
- Seizures.
- Stroke.
In severe cases of hemolytic uremic syndrome, damaged red blood cells can clog the tissue in our kidneys. As a result, our kidneys can’t filter and eliminate waste from our body. If our kidneys can’t filter and eliminate waste, it can lead to acute kidney injury.
Symptoms of acute kidney injury include:
- Decreased urine output (oliguria).
- Hematuria.
- Feeling sick, caused by an increase in blood toxin levels.
- High blood pressure (hypertension).
- Shortness of breath(dyspnea).
- Swelling (edema) in lower extremities, and less often in the face, hands or elsewhere in our body.
Classic triad associated with hemolytic uremic syndrome
A medical triad is a group of three signs that indicate a medical condition.
The classic triad associated with hemolytic uremic syndrome includes damage to the red blood cells (microangiopathic hemolytic anemia), a steady decrease in the number of platelets in the blood (thrombocytopenia) and acute kidney injury.
Cause of hemolytic uremic syndrome
Certain strains of the bacteria E. coli cause most cases of hemolytic uremic syndrome. These strains of E. coli make us sick by producing a toxin called Shiga. Shiga damages the lining of our small intestines and causes diarrhea. It can also enter our bloodstream, destroy red blood cells and damage our kidneys.
Another name for these strains is Shiga toxin-producing E. coli (STEC). The most common STEC is E. coli O157:H7, or E. coli O157.
Certain allopathic medications may cause hemolytic uremic syndrome.
How do I get E. coli?
E coli usually enters our body when we eat spoiled, undercooked or poorly processed foods or drinks. E coli can also spread from person to person through an oral-fecal route.
Diagnosis
After detailed history ask about patient’s medical history and family medical history, conduct a physical exam and order the following tests.
- Urine test. Place a dipstick into the urine, the dipstick will change color if there is protein or blood in urine.
- Blood test.
- Genetic testing.
- Stool test (Stool tests check for the presence of E. coli O157).
- Kidney biopsy. Kidney biopsies are rarely necessary for adults. Children don’t need kidney biopsies.
Allopathic Treatment for Hemolytic Uremic Syndrome
Hemolytic uremic syndrome treatment may include blood transfusions, especially red blood cells and platelets. Red blood cells help with signs and symptoms of anemia, including chills, fatigue, shortness of breath and rapid heart rate. Platelets help our blood clot, especially if a person is bleeding or bruising easily.
Patient may have a greater chance of getting an infection during treatment, so it’s a good idea to tell your patient to wash his/her hands often and distance his self from people with infections, including a common cold or the flu.
Give your patient intravenous (IV) fluids to keep patient hydrated. If patient have severe kidney failure, he may need dialysis to keep his/her blood clean while kidneys heal. Dialysis is usually temporary.
Allopathic medications
If patient have high blood pressure due to kidney failure, advise medications to bring blood pressure down. If patient have atypical hemolytic uremic syndrome, prescribe eculizumab or rovelizumab to help prevent blood clots from damaging blood vessels, blood cells and kidneys.
Eculizumab and rovelizumab increase patient’s chances of developing meningococcal disease and pneumococcal disease, prescribe meningococcal and pneumococcal vaccinations before starting the medications and prescribe antibiotics for at least two weeks.
If patient is pregnant, planning on getting pregnant or breastfeeding before taking eculizumab or rovelizumab. You must be aware your patient about risks to patient and the baby.
Children have an easier time recovering from hemolytic uremic syndrome than adults. More than 85% of people with hemolytic uremic syndrome recover complete kidney function. However, even with full recovery after allopathic treatment, patient will have high blood pressure or other kidney problems in the future.
