Heliotrope rash is caused by dermatomyositis (DM), a rare connective tissue disease. People with this disease have a violet or bluish-purple rash that develops on areas of the skin. They can also experience muscle weakness, fever, and joint aches.
The rash may be itchy or cause a burning sensation. It commonly appears on sun-exposed areas of the skin, including the:
- face (including eyelids),
- neck,
- knuckles,
- elbows,
- chest,
- back,
- knees,
- shoulders,
- hips,
- nails.
It isn’t uncommon for a person with this condition to have purple eyelids. The purple pattern on the eyelids may resemble a heliotropeflower, which has small purple petals.
DM is rare, researchers believe there are up to 10 cases per 1 million adults. Likewise, there are about three cases per 1 million children. Women are more commonly affected than men, and white skin and European, USA and Australians are more commonly affected than Asians and Caucasians.
What causes heliotrope rash?
The rash is a complication of dermatomyositis (DM). This connective tissue disorder has no known cause. Researchers are trying to understand who is likely to develop the disorder and what increases their risk.
Possible causes of dermatomyositis include:
- Family or genetic history: If someone in your family has the disease, your risk may be higher.
- An autoimmune disease: A functioning immune system attacks unhealthy or invading bacteria. In some people, however, the immune system attacks healthy cells. When this happens, the body responds by causing unexplained symptoms.
- Underlying cancer: People with DM are at a higher risk for developing cancer, so researchers are investigating whether cancer genes play a role in who develops the disorder.
- Infection or exposure: It’s possible that exposure to a toxin or trigger could play a role in who develops DM and who doesn’t. Likewise, a previous infection may also affect your risk.
- Complication of medication: Side effects from some medications could lead to a rare complication like DM.
Other symptoms of dermatomyositis
A heliotrope rash is often the first sign of DM, but the disease can cause other symptoms. 
These include:
- ragged cuticles that expose blood vessels at the nail bed,
- scaly scalp, which may look like dandruff,
- thinning hair,
- pale, thin skin that can be red and irritated.
Over time, DM can cause muscle weakness and lack of muscle control.
Less commonly, people may experience:
- gastrointestinal symptoms,
- heart symptoms,
- lung symptoms.
Who’s at risk for heliotrope rash and dermatomyositis?
Currently, researchers don’t have a clear understanding of what factors may affect the disorder and rash.
I (Dr. Qaisar Ahmed) think that it’s a type of varicella zoster infection (Pox- Viral infections). People of any race, age, or sex may develop the rash, as well as DM; DM is twice as common in children between age 2-16 years, in elders the average age of onset is 50 to 70.
DM is a risk factor for other conditions. That means having the disorder may increase your odds for developing other conditions.
These include:
- Cancer: Having DM increases your risk for cancer. People with DM are three to eight times more likely to develop cancer than the general population.
- Other tissue diseases: DM is part of a group of connective tissue disorders. Having one may increase your risk for developing another.
- Lung disorders: These disorders can eventually impact your lungs. You may develop shortness of breath or coughing. According to one study, 45 to 60 percent of people with this disorder develop interstitial lung disease.
How are heliotrope rash and dermatomyositis diagnosed?
If a doctor suspects patient’s rash is the result of DM, he/she may use one or more tests to understand what’s causing the issues.
These tests include:
- Blood analysis: Blood tests can check for elevated levels of enzymes or antibodies that may signal potential problems.
- Tissue biopsy: Your doctor may take a sample of muscle or the skin affected by the rash to check for signs of disease.
- Imaging tests: An X-ray or MRI can help your doctor visualize what’s happening inside your body. This may rule out some possible causes.
- Cancer screening: People with this disorder are more likely to develop cancer. Your doctor may perform a full-body exam and broad testing to check for cancer.
How is Heliotrope rash treated in allopathy?
As with many conditions, early diagnosis is key. If the skin rash is diagnosed early, treatment can begin. Early treatment reduces the risk of advanced symptoms or complications. 
Treatments for heliotrope rash include:
- Antimalarials: These medications can help with the rashes associated with DM.
- Sunscreen: Exposure to sun may make the rash irritated. That may make symptoms worse. Avoid sunscreens, it will not protect the skin.
- Oral corticosteroids: Prednisone is most often prescribed for heliotrope rash, but others are available.
- Immunosuppressants and biologics: Medicines such as methotrexate and mycophenolate, but they can’t help patients with heliotrope rash and DM. That’s because these medicines often work to stop the immune system, said Dr. Qaisar Ahmed.
As DM worsens, you may experience greater difficulty with muscle movement and strength. Physical therapy will not help your patients to regain strength and relearn functions.

Acid Picricum
Acts upon the generative organs probably through the lumbar centers of the spinal cord. Degeneration of the spinal cord, with paralysis. Dorsal pain between shoulders. Ill effects of bruises and shocks to spine. Coccydynia. Dull aching in lumbar and sacral region.
Extremities heavy, weary, paralyzed; trembling; bands unsteady; fingers and toes numb. Muscular weakness, especially of spine and lower extremities. Perspiration of hands.
Causticum
It is one of the most effective medicines for managing cases having progressive loss of muscle strength. It offers benefits where limbs feel heavy and weak. Manifests its action mainly in chronic rheumatic, arthritic and paralytic affections, indicated by the tearing, drawing pains in the muscular and fibrous tissues, with deformities about the joints, progressive loss of muscular strength, tendinous contractures. Broken down senile.
Kyphoscoliosis and scoliosis. Weakness progresses until patient gradually appears paralysis. Kyphoscoliosis and scoliosis. Hemostasis is interfered with, rapid reduction in number of red corpuscles (hence pallor, icterus, anemia). Constrictive sensation in internal organs. Debility, anxiety, emaciation, though appetite and thirst may be excessive. Facial and abdominal muscles twitch. Passive, congestive pain (rises from back of head), with pale face. Head drawn back. Falling of hair; dry and gray. Nosebleed, dark, oozing.
Deformed spinal cord. Spinal irritation, kyphoscoliosis and scoliosis, tingling of lower extremities; can bear only slightest covering. Locomotor ataxia. Formication and numbness. Myelitis. Paralytic weakness; spasms; paralysis. Paralytic weakness; drops things. Ankles swollen. Limbs suddenly give out when attempting to sit down. Dizziness and weakness when descending. Spasmodic twitching of muscles of forearm and hand. Fingers jerk. Parkinson’s diseases. Neuritis. Typewriters’ paralysis.
Veratrum Viride
Tiegel’s contracture, Thompson’s Disease, athetosis and pseudo-hypertrophic muscular paralysis present a symptomatology. Kyphoscoliosis and scoliosis. Difficult breathing. Aching pain in back of neck and shoulders. Severe pain in joints and muscles. Violent electric-like shocks in limbs. Convulsive twitching. Acute rheumatism.
Calcare Flouricum

Angina pectoris; pain especially severe in elbow of left arm. Stitches in heart. Pulse feeble and irregular. Cardiac dropsy with distressing dyspnea. Extremities distended, feel bruised and sore. It is a great medicine for cases where muscles are inflamed and mainly neck muscles are weak. With this pain is also there in neck muscles as if bruised. Gouty pains shoot all through the foot and limb, and in joints, but especially small joints. Swollen, hot, pale. Throbbing in right shoulder. Pressure in shoulder, worse motion. Cracking in joints; worse, warmth of bed. Gouty nodes. Kyphoscoliosis and scoliosis. Rheumatism begins in lower limbs and ascends. Ankles swollen. Soles painful, can hardly step on them. Easy spraining of ankle.
Lycopodium Clavatum
Aneurism. Aortic disease. Palpitation at night. Burning between scapulae as of hot coals. Pain in small of back. Kyphoscoliosis and scoliosis. Numbness, drawing and tearing in limbs. Heaviness of arms. Tearing in shoulder and elbow joints. One foot hot, the other cold. Chronic gout, with chalky deposits in joints. Profuse sweat of the feet. Pain in heel on treading as from a pebble. Painful callosities on soles; toes and fingers contracted. Sciatica. Cannot lie on painful side. Hands and feet numb. Right foot hot, left cold. Opisthotonos – Kyphoscoliosis and scoliosis. Swollen veins of neck. Painless paralysis. Twitching of limbs. Numbness. Jerks as if flexors were overacting. Convulsions; worse from glare of light; coldness of limbs.
Tarentula Hispanica
Weakness of legs; chorea like movements. Numbness of legs. Multiple sclerosis, with trembling. Twitching and jerking. Yawning with uneasiness of legs, must move them constantly. Extraordinary contractions and movements. Kyphoscoliosis and scoliosis.
Phosphorus
Burning in back; pain as if broken. Heat between the shoulder-blades. Weak spine. Use of this medicine is considered in cases in which weakness in upper arms is present. Along with this drawing pan in the muscles of shoulder and upper arm accompanies. In some cases, severe pains in the muscles of the right shoulder are present, and they extend to the elbow. Apart from above it is also used in cases where weakness and heaviness in the lower limbs are present. This is most marked when going upstairs.
Extremities: Ascending sensory and motor paralysis from ends of fingers and toes. Stitches in elbow and shoulder joints. Burning of feet. Weakness and trembling, from every exertion. Can scarcely hold anything with his hands. Tibia inflamed and became necrosed. Arms and hands become numb. Can lie only on right side. Post-diphtheritic paralysis, with formication of hands and feet. Ruta Gravenolense
Acts upon the periosteum and cartilages, eyes and uterus. Complaints from straining flexor tendons especially. Feeling of intense lassitude, weakness and despair. Injured “bruised” bones.

Extremities: Lameness, weakness, trembling and twitching of various muscles. Chilblains. Feet in continued motion; cannot keep still. Large varicose veins on legs. Sweaty. Convulsions, with pale face. Transverse pains, especially in upper extremity. Soles of feet sensitive. Steps with entire sole of foot on floor.
