Fragile X syndrome (FXS), also known as Martin-Bell syndrome, is an inherited condition that causes developmental delays, intellectual disabilities, learning and behavioral issues, physical abnormalities, anxiety, attention-deficit/hyperactivity disorder and/or autism spectrum disorder, among other problems.

The fragile X syndrome is not life-threatening.

It’s the most common form of inherited intellectual and developmental disability (IDD), males are more likely to have fragile X syndrome than females. Their symptoms are also more severe.

Fragile X syndrome is named fragile X syndrome because, when looked at through a microscope, part of the X chromosome looks “broken” or “fragile.”

Fragile X syndrome is one of three syndromes in the fragile X family. The other two syndromes are:

• Fragile X-associated tremor/ataxia syndrome (FXTAS). Symptoms include balance problems, shaky hands, unstable mood, memory loss, cognitive problems and numbness in the hands and feet.
• Fragile X-associated primary ovarian insufficiency (FXPOI). Symptoms include reduced fertility, infertility, missing or unpredictable menstrual periods and premature menopause.

How common is fragile X syndrome?

Although the exact number isn’t clear, researchers estimate that, worldwide, about 1 in 8,000 to 11,000 females and 1 in 4,000 to 7,000 males have fragile X syndrome.

Experts don’t know for sure how many people carry the fragile X premutation, which can be passed down to their child and cause fragile X syndrome.

Parents with the premutation are nearly two times more likely to have a child with a disability than those without the premutation.

Difference between fragile X and autism

Fragile x syndrome can sometimes cause autism. If your child has autism, they have a 2% to 6% chance of also having fragile X syndrome.

Are carriers of fragile X at risk of any health problems?

Yes. If a child has fragile X syndrome, he/she is at a higher risk for:

• Menopause that starts before age 40 (females only).
• Dementia.
• High blood pressure.
• Depression.
• Anxiety.
• Migraines.
• Hypothyroidism.
• Chronic pain.
• Sleep apnea.

Fragile X syndrome gets passed from parent to child. A DNA part called the “CGG triplet repeat” expands within a gene called “FMR1,” producing a mutation that causes fragile X syndrome. A normal part repeats five to 40 times, but in people with fragile X syndrome it repeats more than 200 times. This “silences” the FMR1 gene, which messes up the nervous system and causes the symptoms of fragile X syndrome.

There are four classifications of the fragile X mutation. They’re determined by the number of times the CGG repeats in the DNA:

• Normal.
• Gray zone.
• Premutation carrier.
• Full mutation.

Not everyone with the mutated FMR1 gene has FXS symptoms. Some people are still able to make the FMRP protein because:

• They’re female. Females are not as likely to get fragile X syndrome.
• The size of the mutation is small.
• They have mosaicism. This is where some cells have the FMR1 mutation and others don’t.

Symptoms of fragile X syndrome

Fragile X syndrome affects child’s intelligence, mental health, physical features and behavior. Common symptoms in each category include:

Intelligence issues:

• Learning disabilities.
• Low intelligence quotient (IQ). Their IQ scores decrease with age.
• Delayed early developmental milestones. Milestones are social/emotional, language/communication, cognitive (thinking, learning, problem-solving) as well as movement/physical development (see below).
• Delayed development of nonverbal communication such as using gestures, body language and facial expressions.
• Problems with math.
• Language processing, child struggles with speech and language around age two.

Mental health problems:

• Anxiety.
• Depression.
• Obsessive compulsive behaviors.

Physical features:

• A long, narrow face.
• A large forehead.
• A large jaw.
• Soft skin.
• Large ears and crossed/lazy eyes.
• Very flexible or double-jointed fingers.
• Flat feet.
• Enlarged testicles (in males, after puberty).
• A high-arched palate (the roof of the mouth).
• Low muscle tone.

Behavior:

• Attention-deficit/hyperactivity disorder (ADHD).
• Social anxiety, shyness.

• Sensory disorders – sensitivity to crowds, touch, sounds, foods and textures.

Sometimes patients with fragile X syndrome have other health conditions. In one survey, parents reported that their children also had:

• Seizures (epilepsy).
• Sleep problems. According to a different study, 4 in 10 people with FXS and autism spectrum disorder have sleeping problems. This is compared to 3 in 10 people who have only fragile X syndrome.
• Aggressiveness or irritability. People with fragile X syndrome in addition to autism spectrum disorder are more likely to be aggressive.
• Self-injury behaviors.
• Obesity.

How long does fragile X syndrome last? Is fragile X syndrome curable?

Fragile X is a lifelong syndrome. It can’t be cured.

Diagnosing fragile X syndrome requires DNA from blood, amniotic fluid or other tissues, the laboratory will determine if the child has the FMR1 gene.

Boys are usually diagnosed around 35 to 37 months old, and girls around 42 months. parents might start to notice symptoms in their child as early as 12 months.

If a lady is pregnant and concerned that her child has fragile X syndrome, she can see a genetic counselor where patient may undergo the following prenatal tests:

• Amniocentesis: The doctor takes a sample of the amniotic fluid for testing.
• Chorionic villus sampling: The doctor takes a sample of cells from the placenta for testing.

Great anguish and restlessness. Changes place continually. Fears, of death, of being left alone. Great fear, with cold sweat. Thinks it useless to take medicine. Suicidal. Merc Solubils

Great anxiety, especially when riding in transport, from descending a mountain. Dread of downward motion; child has anxious countenance when laid in cot, or carried downstairs, restless. Calcarea Carbonica

Apprehensive – worse towards evening; fears loss of reason, misfortune, contagious diseases. Forgetful, confused, low-spirited. Anxiety with palpitation. ADHD. Obstinacy: slight mental effort produces hot head. Averse to work or exertion. Fragile X syndrome.

The child remains in constant motion; keeps on running, jumping, climbing here and there; difficult to be seated in one place and is usually impatient. Gelsemium

Vertigo, heaviness of head; band-feeling around and occipital headache. Dull, heavy ache, with heaviness of eyelids; bruised sensation; better, compression and lying with head high.

Hyoscyamus Niger

Hyoscyamus Niger is very effective for Fragile X syndrome. Hyperactivity and is extremely restless. Tends to move from one place to another, and always in urgency. Inattentive and lacks concentration, use inappropriate words and sentences or make up irrelevant answers. Talkative and impatient and may talk absurdly. Some children also tend to make constant unintelligible chattering. Emotional outbursts. The child may cry out suddenly or break into a loud laugh. Violent behavior like – scratching, striking, biting, and injuring those around him, insult, shout, scold, abuse and spit on people.

Ignatia Amara

Changeable mood; introspective; silently brooding. Melancholic, sad, tearful. Not communicative. Sighing and sobbing. Aftershocks, grief, disappointment. ADHD. Fragile X syndrome.

Feels hollow, heavy; worse, stooping. Headache as if a nail were driven out through the side. Cramp-like pain over root of nose. Congestive headaches following anger or grief; worse, smoking or smelling tobacco, inclines head forward. Very sensitive and prone to mood swings, moving from laughter to tears.

Kali Arsenicosum

Scolding, morose, retired, quarrelsome, and discontented, jealous, indifferent to everything, scarcely answered questions addressed to her/him, or replied to them in a peevish tone; eyes had a fixed look, face looked frightened and anxious; feels better every third day. Nervous depression and great nervousness.

Thought her/his head felt larger, headache in left parietal bone, as if it was sore and pressed upon by a hand; behaves like a crazy person. Constricted feeling in head, as if there was a wound on parietal bone which was being scratched; the place feels hot. Fragile X syndrome.

Hypochondria, excessive grooming, and even fear of heart attacks. People with health-based anxiety may have racing thoughts and difficulty sleeping. They may also fear of death or dying; prone to feeling cold and vulnerable to panic attacks.

Stramonium

Hyperactive and restlessness. Patient sits down, rises, kneels, rolls, creeps or takes other strange positions. Continually move hands and arms. Does everything hastily and hurriedly. Inattentive, cannot connect a sentence, is unable to give a proper answer, and cannot find the right words.
Loquacious, disposed to talk continually. Sings, makes verses, shrieks, screams, howls, and groans. He is impulsive and violent too. Tear his clothes, destroys things, and attempt to bite those who come near him. Fragile X syndrome.

Tuberculinum Bovinum

ADHD where the child is hyperactive, restless and always seems to have too much energy, can’t slow down to learn anything and face difficulty while doing tasks that need concentration. Fragile X syndrome. Destructiveness and desire to break things. Irritability, stubbornness, and fretfulness. Argumentative and tend to use foul language, want to fight, and have fits of violent temper tantrums.

Kalium phosphoricum

Fragile X syndrome. Anxiety, nervous dread, lethargy, indisposition to meet people. Extreme lassitude and depression. Very nervous, starts easily, irritable. Brain-fag; hysteria; night terrors. Somnambulance. Loss of memory. Slightest labor seems a heavy task. Great despondency about business. Shyness; disinclined to converse. ADHD.

Occipital headache; better, after rising. Vertigo, from lying on standing up, from sitting, and when looking upward. Cerebral anemia. Headache of students, and those worn out by fatigue. Headaches are relieved by gentle motion. Headache, with weary, empty, gone feeling at stomach.

Morose, irritable; tearful; averse to be talked to; everything is too much; depression. Lethargy. Cannot recall names or words; mind sluggish but will act if aroused. Extreme lassitude and depression; apprehensive. Exhaustion after moderate mental effort. Flies into a passion and can hardly articulate; tongue feels thick. Restless. Nervous; starting. Perverted affections; cruel to husband; to baby. Indifferent and captious.

Giddy when out of doors but feels better when facing sun. Seasick without nausea. Occipital headache, lasting all night; frequent waking, woke with pains in occiput and loins, feels better lying on back, passed off after rising. Sensation of a band round forehead just above eyes.

Phosphorus

Affections of the mind in general; amativeness; dizziness, nymphomania, melancholy and sadness, sometimes with violent weeping, or interrupted by fits of involuntary laughter. Laughs at serious things. Stupor, low, muttering delirium; loquacious. Thinks she/he is several pieces and cannot adjust the fragments. Stupor from which she/he could be aroused for a moment only to lapse back into a muttering lethargy, and forgetfulness. Fragile X syndrome.

Great apathy; very sluggish; dislike to talk; answers slowly or not at all. Anguish and uneasiness, especially when alone, in stormy weather, principally in evening, with timorousness and fright. Anguish respecting the future; or respecting the issue of the disease. ADHD. Susceptibility to fright. Fear in evening, fear of darkness; of specters; of things creeping out of corners.

Hypochondriacal sadness. Disgust to life. Apathy alternating with angry words and acts. Becomes easily vexed and angry, which makes him exceedingly vehement, from which she/he suffers afterwards. Any lively impression = heat, as if dipped in hot water. Great irascibility, anger, passion, and violence. Involuntary and spasmodic weeping and laughter. Misanthropy. Repugnance to labor. Shamelessness, approaching insanity. Great indifference to everything, and even to patient’s own family. Great forgetfulness, especially in morning. Great flow of ill-assorted ideas.

Zoomantic condition; state of clairvoyance. Ecstasy. When anxious or vexed, their thoughts get scattered, and they have a difficult time focusing or getting things done. Their anxiety may be tied to a need for approval in social circles or from romantic partners.

Pulsatilla

This remedy is particularly applicable for complaints which are found to occur in patients of a mild, yielding, or good-natured disposition; also, in those who by, their sickness, or naturally, are very easily excited to tears-they are very apt to burst into tears whenever spoken to, or when they attempt to speak, as in giving their symptoms. Fragile X syndrome.

Affections of the mind in general; covetous; mistrustful; absent-minded; low-spirited. ADHD.  Melancholy with sadness, tears, great uneasiness respecting one’s affairs or about the health; fear of death (tremulous anguish, as if death were near), care and grief.

Involuntary laughter and weeping. Great anguish and inquietude, mostly in precordial region, sometimes with inclination to commit suicide, palpitation of heart, heat, and necessity to loosen the dress, trembling of hands, and inclination to vomit.

Fits of anxiety, with fear of death, or of an apoplectic attack, with buzzing in ears, shivering, and convulsive movements of fingers. Apprehension, anthropophobia, fear of ghosts at night or in evening, with an impulse to hide or to run away, mistrust and suspicion. Covetousness. Taciturn madness: with sullen, cold, and wandering air, sighs, often seated with the hands joined, but without uttering any complaint. Despair of eternal happiness, with continual praying. Discouragement, indecision, dread of occupation, and obstructed respiration. Disposition envious, discontented, and covetous, exhibiting itself in a wish to appropriate everything. Caprice, with desire at one time for one thing, at another time for something else, either being rejected as soon as obtained.

Hysterical laughter after meals. Hypochondriacal humor and moroseness, feels better to evening, often with repugnance to conversation, great sensitiveness, choleric disposition, cries, and weeping. Ill Humor, sometimes with a dread of labor, and disgust or contempt for everything. Inadvertence, precipitation, and absence of mind. Difficulty in expressing thoughts correctly when speaking, and tendency to omit letters when writing. Giddiness: patient neither knows where he is nor what he does. Great flow of very changeful ideas. Nocturnal raving; violent delirium and loss of consciousness. Frightful visions. Weakness of memory. Fixed ideas. Stupidity.

Silica Tera

Despondency, melancholy, and disposition to weep. Nostalgia. Anxiety and agitation; yielding, anxious mood. Taciturnity; concentration in self. Best choice for ADHD where the child shows violent, impulsive and destructive behavior. Strike those around him, tears things up has an impulse to bite.
